12/5/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C engaged Ashiyah and her mother in a conversation to determine how their day was going as well as provide QMHP-C with the any new behaviors that need to be discussed. Ms. Hodge reported she has been having to redirect Ashiyah a lot with her engaging in simple request within the home with behaviors lasting 20 minutes in duration. 2. Behavior: Ashiyah participated in the session and did not need any redirection from QMHP-C. Ashiyah presented some hostility with her mother as the two of them went back and forth about her cleaning the home while there are others in the home who mess things up and she is not going to continue to clean after grown people with her arguing with her mother for 5-10 minutes in duration. 1. Intervention: QMHP-C began the session by inquiring about Ashiyahâs current thoughts, feelings and emotions. QMHP-C redirected Ashiyah throughout the session to focus on ways to deal with the current conflict with her mother and explored with Ashiyah conflict resolution skills. During the family session, QMHP-C explored with Ashiyah and her mother the events that led to their current conflict and encouraged Ashiyah and her mother to utilize problem solving skills to resolve their conflict. 2. Intervention: QMHP-C educated Ashiyah on how her mother wasnât in the wrong with expressing her feelings towards her the behaviors that she is presenting in the home when it comes her being disrespectful towards her as well as her not wanting to complete simple task within the home. QMHP-C educated Ashiyah on how her mother is simply wanting the best for her and she continues to disregard her mothersâ request and continues to do what she wants to do when she wants to do it. 1. Response: Ashiyah appeared to be receptive to QMHP-Csâ interventions and services that were provided to her during the session. Ashiyah indicated to QMHP-C that she just feels as though she is doing everything on her own and she does not have any support from her mother and when she does have support, she has to listen to her mouth so she would rather not have it or she rather not do what she want to do in when it comes to her and the choices she decides she want to make. 2. Response: Ashiyah discussed with QMHP-C that if her mother would just take the time out to listen to what she wants to do and stop making decisions for her then the two of them would get along just fine. Ashiyah further discussed with QMHP-C, âI can clean this house up and down and it is still not to my motherâs likingâ. QMHP-C Interpretation of clientâs progress towards todayâs objectives: Ashiyah continues to struggle with solving conflict. QMHP-C Plan of Action: VACCD staff will continue to encourage Ashiyah to communicate through journaling her feelings and thoughts when feeling sad or depressed 1-2 times weekly.
